Watson confronts Smith and learns that he is an expert on Tapanuli fever because he had previously used it to murder his nephew. Smith had been trying to infect Holmes with the same disease, but Holmes had managed to outwit him. Smith confesses to the murder, believing that Holmes will not live to testify against him.
Watson is horrified by Smith's revelation and rushes back to Holmes' house, only to find Holmes fully recovered and in good spirits. It turns out that Holmes had been faking his illness to trap Smith into confessing to the murder. He had been feigning his illness by using a combination of different disguises and theatrical makeup to make himself appear sick.
As Smith's confession had been made in front of witnesses, the police are called and Smith is arrested. Holmes reveals that he had been aware of Smith's guilt from the beginning and had used his knowledge of crime and human behavior to bring the murderer to justice. The elaborate ruse was necessary to ensure a conviction, as Smith was a highly respected and influential man.
In the end, Holmes and Watson reflect on the case. Holmes explains that he had to deceive even Watson to ensure the success of his plan. He apologizes for the distress he had caused his friend but also emphasizes the importance of his secrecy. Watson, although initially upset, comes to understand and appreciate the brilliance of Holmes' mind and the lengths he is willing to go to for justice.
