K. Scott Griffith’s career took a dramatic turn after he survived a devastating plane crash. While piloting a plane for an airline, he experienced a microburst, a severe aviation hazard, which significantly influenced his career path. With his physics background, he took a leave from his airline job to collaborate with NASA and the FAA, focusing on technologies for predicting and mitigating aviation dangers, particularly microbursts.
Returning to the airline industry, Griffith became the chief safety officer and introduced the Aviation Safety Action Program, or ASAP. This innovative program encouraged aviation professionals to report safety issues without fear of reprisal. The success of ASAP was monumental, leading to a 95 percent decrease in fatal airline accidents.
In the late 1990s, Griffith expanded his focus to health care, applying his safety strategies to this new domain. His approach went beyond traditional quality management, aiming for high reliability in health-care organizations. He emphasized balancing patient expectations, care delivery, and cost while addressing complex interactions within healthcare systems. This strategy contributed significantly to reducing errors and enhancing patient care.
Griffith’s philosophy centered on the concept of high reliability, defined by consistently high performance. This involves two key traits: effectiveness, or achieving desired outcomes under normal conditions, and resilience, or recovering swiftly from difficulties. He advocated for addressing both technological vulnerabilities and human factors like skill deficits and behavioral risks.
Griffith stressed the importance of understanding the complex relationship between people and systems, extending beyond leadership and cultural paradigms. His approach, known as the Sequence of Reliability, focuses on risk management at individual and organizational levels, sequentially addressing systems, humans, and organizational elements, in that order.
The Sequence of Reliability offers a comprehensive framework for risk mitigation and performance enhancement in any organization. It integrates diverse fields such as engineering, psychology, neuroscience, ethics, and law. Applied effectively, this multidisciplinary approach can optimize business processes, prevent disasters, and transform organizational outcomes.
